Yes. This is something I've been seeing quite often recently after returning from tours.
It's called an exhaust tee.
When you exhale, bubbles pass through it and come out from both sides.
For example, when entering from a boat, if it snags on the edge of the boat, it will come off if a certain amount of force is applied.
If you're swimming with your octo dangling underwater 
It might also get caught on coral or rocks and come off if you swim too forcefully.
When it comes off, the inside is completely exposed. It ends up looking like this.
The regulator is designed not to break, so if you're careful not to snag it,
you can avoid losing it 
Don't worry, it doesn't mean your regulator is unusable!
Unless it's a very old model, you can purchase it as a replacement part.
Next up is this. By the way, I (Mii-man) have lost this before too 
The weight pocket on a weight-integrated BCD.
This, you see... if it's not properly attached, or if the lock is stiff and doesn't engage correctly,
when you look down underwater... WHOOSH.
If the weights fall out, it can lead to a rapid ascent, so always make sure it's securely fastened beforehand.
Also be careful not to leave it behind when drying it 
These are parts that are often lost without realizing it.
If your equipment breaks down, it also leads to expenses so if you pay more attention to how you handle your equipment,
you'll avoid breakages and losses
